What are session notes in ABA?

Session notes are the clinical documentation completed after each ABA session, summarizing the interventions used, the learner's responses, progress toward goals, and any notable observations.

Session notes are one of the most common documentation tasks for RBTs and BCBAs. Each note records what actually happened: the programs run, the strategies and prompts used, how the client responded, and anything worth flagging for the supervising clinician.

They serve two purposes at once. Clinically, they give the BCBA the context behind the numbers. Administratively, they are the record that supports the claim - payers expect the note to match the service, date, duration, and provider being billed.

Because a note is required for every session, the time they take adds up quickly. Writing them alongside data capture, rather than reconstructing them at the end of the day, keeps them accurate and keeps billing moving.

Most payers expect a consistent set of elements: the date and start and end times, the service delivered and where, who delivered it and their credential, the goals worked on, the interventions and prompting used, how the client responded, and a signature. Requirements vary by payer and state, so practices generally build their note template around the strictest contract they hold rather than maintaining several formats.

The common failure is a note that is technically present but says nothing specific. Wording that could describe any client on any day - worked on goals, client did well - does not demonstrate that a skilled service was delivered, and it is exactly what an auditor looks for. A defensible note ties what happened to the client's actual targets and reads differently from the one before it.

Timeliness matters as much as content. Notes written days later are less accurate, delay claim submission, and in some contracts fall outside the window the payer allows. Because the note also has to agree with the units billed and the data recorded for the same session, capturing it while the session is still fresh is the simplest way to keep those three records from contradicting each other later.
